The order matters. We test before we treat. Symptoms alone are a poor guide here, because fatigue, poor sleep, low mood and weight gain look identical whether the cause is thyroid, iron, vitamin D, sleep apnoea, depression or a genuine hormone deficiency. Lab work separates them, and it also gives us a baseline to measure treatment against.

Testosterone declines gradually in men from around the fourth decade, a process usually called andropause. The symptoms men report most are flattening energy, loss of muscle despite unchanged training, more central body fat, disturbed sleep, low mood and reduced libido.
Treatment is only appropriate where testing shows a genuine deficiency and where the risks have been discussed. Testosterone therapy is not suitable for everyone, it requires monitoring, and it has effects on fertility that any man who may want children should understand before starting. That conversation happens before a prescription, not after.

Hormones are not the answer to everything. Where the labs point at thyroid disease, iron deficiency, a sleep disorder or something else entirely, we treat that instead. Our primary care and weight management services often run alongside.
Hormone therapy is not a weight loss treatment, an antidepressant or a substitute for sleep. It can make a genuine difference to energy, sleep quality, mood and body composition where a real deficiency exists, and it does very little where the deficiency does not. It also does not remove the need for the ordinary things: sleep, resistance training, protein, alcohol intake and stress all continue to matter, and in some patients they matter more than the prescription.
Be wary of any provider who diagnoses you over the phone, treats without testing, or promises a specific result. Hormone care is a monitored medical treatment with real risks and real benefits, and it should be presented that way.
