When you submit the contact form we collect the name, phone number, email address, request type and any message you provide, together with the page you submitted from and the time of submission. Our web server keeps standard access logs.

The contact form is not a secure channel and should not be used to send medical or health details. We take your history at your appointment.

How we use it

Submissions are used to respond to your enquiry and to arrange an appointment. They are emailed to the clinic and stored in the website administration area. We do not sell your information, and we do not share it with third parties except the service providers that deliver our email and host this site.

Retention

Form submissions are kept for as long as needed to respond and to maintain a record of enquiries. You can ask us to delete a submission by calling the clinic.
